IVF vs. IUI: Which Fertility Path is Right for You?
Medical Review by Jivanjeet Clinical Advisory Board — Updated June 2026
Executive Summary
When starting fertility treatment, couples are typically faced with two core options: **Intrauterine Insemination (IUI)** and **In Vitro Fertilization (IVF)**. IUI is a simple, low-cost procedure that prepares and injects sperm directly into the uterus. IVF is a complex, high-technology multi-step process where egg retrieval and fertilization happen in a specialized laboratory.
Side-by-Side Comparison
| Feature | IUI | IVF |
|---|---|---|
| Invasiveness | Low. Office procedure; no sedation. | High. Involves surgical egg retrieval under sedation. |
| Average Cost (India) | ₹15,000 — ₹35,000 per cycle. | ₹1.5 Lakh — ₹2.5 Lakh per cycle. |
| Success Rate (Avg) | 10% — 15% per cycle. | 40% — 55% per cycle. |
| Timeline | ~2 weeks per attempt. | 6 to 8 weeks (retrieval to transfer). |
| Prerequisites | At least one open fallopian tube; reasonable sperm. | Bypasses fallopian tubes; works with severe male factors. |
Understanding IUI (Intrauterine Insemination)
IUI, often referred to as artificial insemination, is designed to give sperm a "head start." The woman takes mild ovulation medication (pills or low-dose injections) to stimulate the growth of 1 to 2 follicles. When the eggs are mature, a trigger shot is given to initiate ovulation.
On the day of the procedure, the male partner provides a semen sample. The laboratory washes the sample to concentrate the healthiest, most motile sperm. Using a thin, flexible catheter, the gynecologist deposits this concentrated sperm sample directly into the uterine cavity, bypassing the cervix.
Understanding IVF (In Vitro Fertilization)
IVF is a multi-step technology that takes place outside the body. It consists of four distinct phases:
- Ovarian Stimulation: The female partner receives daily hormone injections for 10–12 days to stimulate multiple follicles (ideally 8–15).
- Egg Retrieval: Eggs are retrieved from the ovaries via a needle guided by ultrasound, performed under light sedation.
- Fertilization & Development: Eggs and sperm are fertilized in the laboratory and grown in incubators for 3 to 5 days to reach the blastocyst stage.
- Embryo Transfer: The healthiest embryo is placed back into the woman's uterus. Extra embryos are cryopreserved (frozen) for future cycles.
Decision Guidance
While IUI is cheaper and less stressful, its success rates are cumulative. If you have blocked tubes or severe male factor issues, IUI will not work. Under FOGSI guidelines, if a couple has attempted 3 stimulated IUI cycles without success, they should proceed to IVF, as additional IUI cycles rarely result in conception.
